The Essential Sequence

Before micropulse cyclophotocoagulation (MPC), Cassie retained useful vision in her left eye, approximately 20/100, despite severe glaucoma and complex ocular anatomy.

Following MPC in February 2024, her vision deteriorated dramatically to hand-motion vision (HM).

At an important postoperative examination:

• Visual acuity had deteriorated to HM.
• IOP was approximately 5 mmHg.
• Posterior visualization was inadequate.
• Dense posterior capsular opacity (PCO) was documented.
• YAG capsulotomy and dilated examination were planned.

Her intraocular pressure subsequently recovered, but her vision did not.

YAG treatment addressed the PCO, yet profound visual impairment persisted.

The records presently assembled do not establish when or how the anatomical cause of that permanent visual loss was definitively identified.

The Central Question

When should the anatomical cause of Cassie’s abrupt and profound visual loss have been established, what diagnostic investigation was indicated, and was the investigation actually performed in a timely and appropriate manner?

A second, separate issue concerns subsequent Pred Forte/prednisolone prescribing, medication reconciliation, monitoring and refill history.
